Our outreach team includes Yve Humphreys, outreach coordinator and Yvonne Conlon, outreach worker. The team support families living within the catchment area for Clapham Manor and Heathbrook Children’s Centres.
They are always on hand to offer support and advice on various of subjects including:
- Mental health and wellbeing
- Relationships and domestic abuse
- Finding work, training or volunteering opportunities
- Finding childcare
- Child development and behaviour
- Housing issues and homelessness
- Benefits, debt and food vouchers
- Parenting support
Families requiring outreach support can self refer or be referred by one of our partner agencies i.e. Nurseries, G.P's, Health Visitors or Social Workers. Support for Children with Special Education Needs or Disability (SEND)
We are committed to supporting families with children with SEND. At Clapham Manor Children’s Centre we have a weekly session especially for children with complex needs supported by highly specialised speech and language therapists.
Fatemeh Mozaffari- Chinjani is our Early Years SENCO. She is available to offer advice, support and referrals to families who have children with SEND. You can contact her through the centre by email, phone or telephone. She works closely with the outreach team to offer holistic support.
